A late degree recognition, an unsigned academic certificate or a fee paid short can keep you out of the MIR. This page sets out the admission rules of the current call, Orden SND/854/2026 (published in the BOE on 10 August 2026, for training posts starting in 2027). Each call opens a short application window, announced in the BOE order that convenes it, and there is no late window once it closes. If you have applied, the lists section below is your next step.
The rule that sits above all the others
Every admission requirement must be met and proved within the application window, and must still hold throughout the tests and, if you win a post, until you join it. If the Ministry later finds a requirement was not met, the Dirección General de Ordenación Profesional can exclude the applicant after hearing them.
Who can apply
The degree
For medicine, you need a qualification that entitles you to practise as a doctor: a Spanish official degree (you must have earned the right to it, not necessarily received the certificate), or a foreign degree recognised, homologated or declared equivalent in Spain, with that right or decision obtained before the window closes. A pending homologation file does not count. Homologation and equivalence are granted by the Ministerio de Ciencia, Innovación y Universidades; qualifications from other EU states are recognised under Real Decreto 581/2017.
Nationality
The order opens the test to:
- Spanish nationals and nationals of the other states covered by article 57 of Real Decreto Legislativo 5/2015 - this covers EU citizens (the document rules also name EEA states, Switzerland and Andorra), certain family members of Spanish or EU nationals who hold an EU family-member residence card, and foreign nationals with legal residence in Spain (passport plus a residence Tarjeta de Identidad de Extranjero).
- Stateless persons - on equal terms, under Real Decreto 1155/2024.
- Nationals of other countries - for medicine, pharmacy and nursing only, if their country has a cooperation agreement with Spain that is signed and in force during the window, and they hold the recognised or homologated degree. On the same terms, nationals of those countries who are in Spain with a lawful short-stay, study or other recognised stay may also apply.
The order does not list the agreement countries, so confirm yours with the Ministerio de Sanidad. In medicine, up to 968 of the 9,676 places may go to people who do not hold a Spanish residence authorisation.
Age
You must not have passed the legal compulsory retirement age, and must not reach it before the end of the maximum residency period for your specialty.
Spanish at C1
Nationals of countries where Spanish is not an official language must prove Spanish at C1 or above (Common European Framework) with an authentic copy of one of:
- a DELE diploma at C1 or above, or a certificate from the exam centre showing you were declared apto on the Instituto Cervantes results;
- a certificate showing you have gained the competences of the specialised courses (cursos especializados) at C1 or above, under Real Decreto 1041/2017 (Spain's official language schools);
- a diploma in the SICELE system with an accredited C1-equivalent level;
- a SIELE certificate showing C1 equivalence in each of its four tests.
You are exempt if your degree was taught in Spanish at a university in Spain or another Spanish-speaking country, or if you have completed at least two full years of specialist training in Spain.
Current residents
If you already hold a training post, you must renounce it before the window closes.
Work authorisation after allocation
Non-EU nationals who win a post must hold, before taking it up, the authorisation to live in Spain and carry out specialist training, under Ley Orgánica 4/2000 and Real Decreto 1155/2024. Every allocated resident must also pass a medical examination, join the professional college where a state law requires it, and provide a clean certificate from the Registro Central de Delincuentes Sexuales.
The fee and the exemptions
The exam fee (derechos de examen) for medicine is €31.10, as published in the order on 10 August 2026. It is paid to the Spanish Treasury:
- Online application - registering electronically includes electronic payment, and the full reference number (NRC) must appear on the form.
- In-person application - you pay at a collaborating bank before registering, and the bank's validation must appear on the form.
- Through a Spanish embassy or consulate - you may pay by transfer from a bank outside Spain to the account in the order (copy the SWIFT and IBAN from the BOE text): one transfer per applicant, with your full name and the concept «tasa prueba selectiva», and the receipt attached. Bank charges are yours.
If you cannot prove payment within the window, or you paid less than €31.10, you are not admitted. The fee is not refunded if you are not admitted for a reason attributable to you.
You are exempt if you prove one of these:
- Victim of terrorism - an authentic copy of that status under Ley 29/2011.
- Large family (familia numerosa) - the official title under Ley 40/2003.
- Recognised disability of 33% or more - the decision recognising the degree of disability.
- Unemployed - a certificate from the Servicio Público de Empleo Estatal or the regional equivalent.
By applying, you let the Ministry check unemployment, disability and large-family status automatically unless you tick the opt-out box. If you opt out, or your large-family title is from Ceuta or Melilla, attach the document yourself; it can also be supplied in the correction period.
Documents to upload
Everything is attached as PDF with the application:
- Degree - a copy of the Spanish degree, the recognition, homologation or equivalence decision, or the degree proof from the Carpeta Ciudadana. A provisional substitute certificate is accepted for Spanish degrees. A bank receipt for the degree fee is not. If you were admitted in an earlier call, you can say so instead of resending the degree.
- Foreign nationality - EU and similar nationals: a valid passport or ID card plus, where applicable, the Registro Central de Extranjeros registration certificate or the permanent-residence certificate; spouses and descendants who qualify as family members also attach the EU family-member residence card (tarjeta de residencia de familiar de ciudadano de la Unión). Others: passport plus Tarjeta de Identidad de Extranjero for a residence or stay, passport alone on the agreement route with no residence or stay, or the statelessness card.
- Spanish C1 proof - if the language rule applies to you.
- Exemption proof - if you claim an exemption and it is not checked automatically.
- Exam-day adaptations - a reasoned request plus a medical certificate, under Orden PJC/804/2025, sent within the window.
- Academic certificate - showing completion date, subject grades and an overall average on a base of 10, electronically signed by the university. Screenshots, campus printouts and documents without an electronic signature or verification code are not accepted.
- Foreign study record - the declaration of equivalence of your average grade from the Ministerio de Ciencia, Innovación y Universidades (requested on its website and signed by you), together with the certificate from your original university.
A missing or invalid academic certificate does not exclude you, but your academic average is set at 5.00.
How the application works
You complete the model 790 form on the Ministerio de Sanidad sede electrónica, under the procedure for the annual FSE selection tests (SIA code 2897569). Each form has a unique reference, copies are invalid, and handwritten forms are never processed.
- With a DNI or NIE - electronic registration is compulsory, identifying with a recognised electronic certificate or Cl@ve Permanente and signing with AutoFirm@ or Cl@veFirma.
- Without a DNI or NIE - print the form's three copies, sign each by hand, pay first, then lodge it at a Spanish embassy or consulate, a registry assistance office in Spain or a Spanish post office.
- Changes - before the window closes, submit a new application by the same route; the latest counts, with no second fee if you prove the first payment.
- Exam city - you choose from 26 cities in Spain, and the choice cannot be changed after the window closes except in documented exceptional circumstances approved by the Dirección General de Ordenación Profesional.
- Disability turn - once chosen, the turn cannot be changed, except to correct a genuine error during the correction period.
- Email - the address on the form is used for notices, and errors in it are your responsibility.
Admitted and non-admitted lists
After the window, the Ministry publishes the provisional lists on its website, showing your name, file number, pseudonymised ID, disability turn, exam city, status and any reasons for non-admission.
You then have ten working days, from the day after publication, to correct the problem or send what is missing. If you do not, the non-admission stands.
The definitive lists follow, adding your exam table (mesa) and centre. Publication counts as notification, and any claim not reflected is treated as rejected. You may lodge a recurso de alzada with the Secretaría de Estado de Sanidad within one month from the day after publication. The order sets no calendar date for the provisional lists, so watch the Ministry's specialist-training pages.

Can I apply for the MIR while my homologation is pending?
No. The recognition, homologation or equivalence must be obtained before the application window closes.
How much is the MIR application fee?

Can a non-EU doctor apply for the MIR?
