The real USMLE Step 1
USMLE Step 1 assesses whether you can apply the integrated basic sciences to the practice of medicine. It is a one-day, computer-based exam of single-best-answer clinical-vignette MCQs. From 14 May 2026 the test day is fourteen 30-minute blocks of up to 20 questions each (a maximum of 280 items) across roughly eight hours, with a 45-minute break pool and optional tutorial time. Content is organized as a matrix: the basic-science disciplines -- biochemistry and molecular biology, genetics, microbiology, immunology, pharmacology, pathology, physiology, and behavioral science -- crossed with the organ systems and the general principles, plus cross-cutting communication, ethics and biostatistics. Since January 2022 the outcome has been reported as Pass/Fail only, with no 3-digit score, which has shifted differentiation toward Step 2 CK; Step 1 is now a pass-the-standard hurdle. Step 2 CK and Step 3 are separate examinations and are not covered here.
What the real USMLE Step 1 covers
Practice is organized around the Step 1 content matrix. The General Principles disciplines -- Biochemistry & Molecular Biology, Medical Genetics, Cell Biology & Histology, Microbiology, Immunology, General Pharmacology and General Pathology -- are practiced first, then applied across the organ systems: Nervous System & Behavioral Science; Musculoskeletal, Skin & Connective Tissue; Cardiovascular; Respiratory; Gastrointestinal; Renal & Urinary; Reproductive; Endocrine; Hematology & Lymphoreticular; and Multisystem, Nutrition & Aging. Two cross-cutting areas -- Communication, Ethics & Patient Safety, and Biostatistics & Epidemiology -- round out the blueprint. Most items are text-only single-best-answer vignettes with five options; a small set of image modules (normal histology, histopathology and gross pathology, microbiology micrographs, and ECGs) test figure interpretation.
How people prepare for it
- 1Build the discipline foundations first -- biochemistry, microbiology, immunology, pharmacology and pathology recur across every organ system.
- 2Practice mechanism-first: for each vignette, name the diagnosis, then the enzyme, mediator, receptor, microbe or pathophysiology behind it.
- 3Rotate through the organ systems in blocks so systems-by-process integration becomes automatic.
- 4Do not neglect the cross-cutting areas -- ethics, patient safety and biostatistics are reliably tested and easy to under-prepare.

How the real USMLE Step 1 is reported
Step 1 is reported as Pass/Fail only -- there has been no 3-digit numeric score since January 2022. There is no penalty for wrong answers, so attempt every question. The pass standard is set by the USMLE program (recent minimum-passing equivalent around the low-190s on the former scale); the real pass decision is made by the USMLE program after your sitting.
